REGISTRATION FORM

Please download and fill out this form to register for camp- the form may be mailed in for early registration** or may be brought inon the first day of camp. To learn more about camp fees and discount please visit our rates and fees page.

HEALTH FORM

The health form is required for a camper to participate at camp according to state law and regulations, but the health form is not required for the early registration discount** and can be given to the camp nurse on the first day of camp.

A physical* and a signature from a physician is required for the full completion of the health form. Unfortunately, due to state law and regulations we cannot admit a camper without a completed health form.

**In order to receive the early registration discount your registration form must be postmarked two weeks prior to the start of camp and the $40.00 deposit fee must be included with the registration form- visit our rates and fee page for more information.
